Guam - The Department of Public Health and Social Services (DPHSS), in conjunction with the Guam Department of Education (GDOE) will be conducting the first school based H1N1 mass vaccination at Marcial Sablan Elementary School on Tuesday, November 24th. The elementary school is the first of four schools selected to participate. 
Parental Consent Forms and Information Packets were distributed to students at Marcial Sablan Elementary on Wednesday, November 18 and the Parental Consent Forms must be returned to the school with the parent’s signature no later than Friday, November 20. According to the DPHSS and GDOE, students who do not return a signed consent form will not be vaccinated.
 
There are four schools identified for initial participation in the school based H1N1 mass vaccination.  They are Marcial Sablan Elementary, Agueda Johnston Middle, Southern High, and Tamuning Elementary schools. Once these vaccination sites have been completed, vaccinations at the remaining schools will commence with dates to be announced later. The dates and times for all school based vaccinations are subject to change, based on vaccine availability
